Person-centred support
Focused on quality of life
About Us
Person 2 Person Services is a registered NDIS provider supporting children and adults with complex disabilities to reach their goals and live well. We partner with participants, families, and care teams in homes, schools, and community settings to provide person-centred, evidence-based support. Our qualified team is committed to delivering respectful, safe, based and high-quality services in line with NDIS standards.
Our Services
Positive Behaviour Support (PBS)
Positive Behaviour Support (PBS) is a person‑centred, evidence‑based approach that focuses on understanding why behaviours occur and building practical skills, routines, and environments that improve everyday life. Rather than simply trying to stop behaviours of concern, PBS enhances quality of life for the person and those around them through plans tailored to individual strengths, preferences, and goals.
For families, PBS provides clear, realistic strategies that reduce stress at home, strengthen relationships, and build confidence to respond safely and calmly. For support workers and providers, PBS offers structured, proactive approaches that reduce incidents, support staff wellbeing, and align with NDIS quality and safeguarding expectations.
TelePBS
TelePBS (Telehealth Positive Behaviour Support) is a remote service delivery model that provides individuals, families, and caregivers with access to expert behaviour support via video conferencing, phone calls, and digital resources. This approach ensures that high-quality, evidence-based support is accessible, flexible, and tailored to individual needs.
Specialist Support Coordination
Our Specialist Support Coordination service assists participants with justice system involvement and higher or complex needs to implement their NDIS plans and achieve a successful transition into the community. We work alongside participants while they are in prison to understand their plans and post-release support needs, and collaborate with prison social workers to prepare for release, source suitable housing, and arrange a support team experienced in complex needs.
In the community, we maintain close communication with community corrections officers to support compliance with parole and home detention obligations, including weekly sign-ins and related requirements. We also link participants with services that address criminogenic needs, including alcohol and other drug services, financial counselling, mental health supports, and other specialist providers, delivering a coordinated and person-centred approach.
Social Work Support and Therapy
We provide tailored social work support and therapy for NDIS participants involved with the criminal justice system. Our approach is person-centred and focused on your goals, values, and circumstances, helping you build self-awareness, develop coping strategies, strengthen resilience, and reconnect with community. We offer support for anxiety, trauma, stress, grief, and life transitions.
